Alliance for Better Care are advertising on behalf of Langley Corner Surgery - Crawley Care Collaborative PCN, who are looking for an experienced Advanced Nurse Practitioner.
Whilst General Practice has somechallenging times ahead, this role provides an exciting and innovativeopportunity for someone who wants to make a real difference in the communitiesthey serve. The successful candidate will be provided with excellent support,the opportunity for development in the role, and will be clinically supervisedby a General Practitioner.
Main duties of the job
The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC requirements for advanced practice must be met.

Our Primary Care Network builds on the existing primary careservices and enables a greater provision of proactive, personalised and moreintegrated health and social care. We are supported by practitioners inadditional roles who allow us to create bespoke multi-disciplinary teams basedon the needs of our local population. Assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis. Clinically examine and assesspatient needs from a physiological and psychological perspective.
Assess,diagnose, plan, implement and evaluate interventions/treatments for patientswith complex needs. Proactively identify, diagnose and manage treatment plansfor patients at risk of developing a long-term condition.
Diagnoseand manage both acute and chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan.
