Job summary The Frailty Care Co-ordinator will have a key role in working with the community services team and wider primary care team to ensure frail patients in the community receive appropriate interventions to support them to live as independently as possible. This will involve supporting the administrative co-ordination of a range of clinicians and representing the team at daily frailty meetings. The role will include data collection, reporting and liaising directly with clinicians to inform workloads. Main duties of the job The post-holder will, under the oversight of the PML clinical practitioner and reporting to the Operations Support Manager be responsible for the following: Daily attendance at the frailty multidisciplinary team meeting Attend weekly frailty MDT meeting Assist with the coordination of referrals for our community teams.
