Detailed job description and main responsibilities
Frailty Strategy overview
We recognize that frailty is associated with increased ill health, mortality, healthcare use, care dependency, and reduced quality of life. We have identified that a high proportion (40%) of our acute medical admissions are frail. By undertaking a comprehensive geriatric assessment on arrival, we can minimize the time spent in hospital and reduce unnecessary admissions.
