An exciting opportunity has arisen for a Pharmacy Technician to join the Community Agile Pharmacy Service (CAPS). CAPS plays a significant role in supporting patients following an acute hospital discharge or to prevent avoidable admissions during periods of acute illness.
The team supports staff and patients within Active Recovery, Community Nursing, Virtual Wards, Community Therapy and the Community Stroke Service.
Main duties of the job
The successful candidate will work closely with patients, relatives and carers to help optimise individualised medication regimens. Our goal is to support patients to reach an appropriate level of independence with their medicines and to support healthcare professionals gain insight into medicines management as part of a holistic assessment process.

Person Specification
Qualifications
* NVQ L3 and/or BTEC in Pharmaceutical Sciences or equivalent
* GPhC Registered
* Accuracy Checking Technician course or willing to work towards
* Evidence of formal teaching course
Experience
* Extensive experience in primary, secondary or intermediate care
* Experience of undertaking 0-2 medication reviews
* Previous experience of patient counselling
* Previous experience of providing patient focused medicines management services
* Good clinical knowledge of medication management relevant legislation and good practice related to storage and handling of pharmaceuticals.
* Up-to-date pharmaceutical and medicines management knowledge
* Understanding of medicines related issues in intermediate care
* Able to supervise junior staff and assess competency and best practice
* Experience of managing conflict with occasional aggressive patients and relatives
* Experience in community services, community pharmacy or primary care
* Experience of audit and project work
Further Training
* Evidence of Continuous Professional Development via personal development plans/file
* Certificate in professional development for pharmacy technicians
Special Skills
* Self-motivated with ability to prioritise work
* Able to identify, analyse and interpret individual patient medication issues
* Able to compare medication management options and make recommendations as appropriate
* Able to devise individualised medicine adherence solutions following patient assessment
* Able to optimise medication regimes to reduce social care costs and maximise cost effectiveness
* Able to work independently on own initiative and manage workload
* Ability to work as part of a multi-disciplinary team
* Able to develop, produce and deliver training packages
* Excellent organisational and analytical skills
* Excellent communication, interpersonal skills
* Computer Literate, including Word, Internet and Excel
* Knowledge of community pharmacy, role of general practice, role of early discharge pathways, roles of other NHS primary care health professionals, and local NHS structure.

* The duties of this post require travelling throughout the city on a daily basis; therefore the post holder must be independently mobile. (Adjustments can be considered in accordance with the Disability Discrimination Act)
* Ability to manage an unpredictable workload with frequent interruptions
* Able to work in sometimes unpleasant environments in the community
* Willingness to work flexibly including working outside of traditional normal working hours in accordance with service need

Salary: £29,970 to £36,483 a year (Per annum pro rata for part time staff)
