We have an exciting opportunity for Band 5 registered nurses to work within Longmoor House.
Mersey Care NHS Foundation Trust and Liverpool University Hospitals NHS Foundation Trust have worked in partnership to develop a brand new physical health service to improve patient outcomes. Longmoor House is a newly refurbished Integrated Care Unit that provides step down capacity from acute trusts for medically optimised patients where it is anticipated that there are ongoing nursing, therapy and reablement needs.
In joining the team you will have the opportunity to:
1. Work in a supportive and caring environment
2. Have the opportunity to develop professionally
3. Deliver great healthcare and make a positive difference in patients’ lives
4. Work within an integrated team with all members of the MDT and partner services
5. Work in a state of the art newly refurbished facility, utilising technologies to improve patient care
The service is based on a Nursing and Therapy reablement model and will focus on supporting individuals within a dedicated integrated adult service. We are looking for qualified RGN Staff Nurses to join this exciting service development which is aimed at supporting a holistic recovery model. Our combined expertise, revised pathways and community connections will enable more timely discharge in a person centred way.
