Job summary We are offering a rewarding and unique development opportunity for a Band 7 Trainee Advanced Clinical Practitioner (ACP) join our team to work across both our Crisis Assessment Suite and Health-based Place of Safety (136 Suite) based in our newly developed facilities at St George's Hospital, Stafford. This development post provides you with the opportunity to progress from a Band 7 to a Band 8a on successful completion of the MSc Advanced Clinical Practitioner Training. You will be fully supported to maximise your potential during your training period. You will have responsibility in providing high quality evidence based person-centred physical and mental health care to Individuals within the Crisis Assessment Suite and 136 Suite and provide effective leadership to the wider multi-disciplinary team in the delivery of high quality, safe and effective care to services users. Both services operate 24/7 provides specialised facilities which provides a safe and comfortable environment where a person with receives immediate assessment and support to those that are experiencing a mental health to see how they can best be supported. Due to the nature of this role, there may be some expectation to work some weekends and evenings as and when required. We can consider requests for flexible working in line with the requirements of the role. Main duties of the job As Trainee Advanced Clinical Practitioner you will receive patients with undifferentiated and undiagnosed problems and make an assessment of their health care needs, based on highly-developed nursing knowledge and skills, including physical examination skills. As part of this role, you will be expected to: Plan, develop and manage specialist packages of care, employing the Care Programme Approach (CPA), working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use. Plan and coordinate the physical healthcare needs of service users in the Crisis Care and136 Suite; including the requirement for annual physical examinations and investigations. Order necessary investigations, and provide treatment and care both individually, as part of a team, and through liaison with / referral to other agencies, e.g. ECGs, Clozaril, Bloods. Draw on a diverse range of knowledge in making decisions to determine evidence-based therapeutic interventions (which will include prescribing medication and actively monitoring the effectiveness of therapeutic psychological interventions). Use professional judgement in managing complex and unpredictable care events and capture the learning from these experiences to improve patient care and service delivery. Undertake regular assessments of all service users under their care, addressing issues and assessing and managing risks identified.
