Main duties of the job
We offer brilliant support for our Newly Qualified Social Workers with a comprehensive induction and a 12-month ASYE programme, with dedicated time off the job each month in your first year to focus on development and wellbeing. Adult Community Team South is a large and supportive team consisting of Social Workers, Occupational Therapists, and Social Care Practitioners. We work with adults in need over the age of 18, actively listening, collaborating, and tailoring plans to enhance well-being. We partner with healthcare providers to prevent crises, emphasizing existing support for independence. Our approach seeks local, sustainable solutions and offers diverse intervention and prevention opportunities.
We’re looking for Newly Qualified Social Workers who have strong interpersonal and effective communication skills, with updated knowledge of relevant legislation and safeguarding vulnerable adults.

Job responsibilities
1. Fully engage in the Assessed and Supported Year in Employment (ASYE) programme for newly qualified social workers by planning and reviewing progress and development at regular intervals, fully cooperating with the ASYE assessor and the supervisor (where these are different people), attending support groups and other activities for NQSWs and preparing for own supervision and appraisal through self-evaluation, ensuring that continual professional development is evidenced.
2. Undertake person-centred social work/community care assessment with customers and plan for the provision of support and/or services with customers in order to assist them meet their outcomes and to live as independently as possible, fulfilling their individual potential.
3. Develop creative approaches to meeting needs and develop up-to-date knowledge of local provisions to ensure this is included, wherever possible, in order to meet identified need within available resources.
4. Have responsibility for accurate and timely recording of all information/assessment activity onto the Social Care records database in accordance with work targets set by the Team Manager and Supervisor.
5. Apply assessment and the management of risks for individuals and apply Council policy to support customers in managing risk appropriately. Identify where vulnerable adults may be at risk and act in accordance with safeguarding policies and procedures.
6. Gather sufficient information about customers in order to accurately assess their needs against eligibility criteria. Develop up-to-date local knowledge about universally available services, including what is available within the voluntary and community sector, as well as the service areas commissioned and contracted services and signpost/advocate for customers accordingly.
7. Convene meetings e.g., core groups to assist in the implementation and review of plans for vulnerable adults.
8. Following a period of induction, participate in the Specialist Response rota enabling access to services to work effectively, making decisions about risk, eligibility, and priority.
9. Ensure that Council corporate and service policies and procedures are followed at all times.
10. Ensure that Support Plan Reviews are carried out in a timely and person-centred way, as required and identified by departmental targets. Ensure agreed outcomes have been met and evidence the need for continuing support at all times in accordance with eligibility criteria, working with customers and care providers to identify the promotion of increased independence.
11. Following a period of induction, contribute proactively to the effective working of the team with a positive attitude, by sharing knowledge, offering advice and support, and by preparedness to be involved in the development of services.
12. Be an ambassador for the Council and the department at all times, representing the Council positively, professionally, and appropriately at meetings with customers, external partners, and agencies. 13. Attend Civil Emergencies when a Rest Centre is set up.
14. Successfully complete the ASYE programme.
15. Ability to travel on a daily basis and undertake visits to customers in their own homes or in other settings e.g., hospital.
Person Specification
Qualifications
* Qualified Social Worker, taking up post within one year of achieving a recognised social work qualification.
* Social Work England registered.
* Able to carry out the following, at a level appropriate for a NQSW:
* Up-to-date knowledge and understanding of relevant legislation and guidance in relation to working with and the safeguarding of vulnerable adults.
* Knowledge and understanding of Health & Social Care legislation and wider policy context.
* Well-developed interpersonal skills and ability to effectively communicate with people in a variety of ways and levels.
* Ability to utilise a range of Microsoft and Electronic Social Care record software.
* Ability to work with others in a developmental way.
* Must be able to write clear, complex, and accurate reports.
* Evidence of appropriate work experience and reflective learning from placement experience whilst qualifying as a Social Worker and of appropriate professional and personal development relevant to length of post-qualifying experience.
* Ability to identify problems with some complexity and to demonstrate a creative approach to such problems.
* Ability to develop and maintain effective professional relationships within and outside the department.
* Fully fluent in English.
