We are a highly supportive training practice with a long-standing team including GPs, Advanced Clinical Practitioners, Nurse Practitioners, Practice Nurses, HCAs and many more additional roles.

Job responsibilities
Job summary
Responsible for supporting the Practice in implementing effective medicine management across the patient demographic, identifying areas for improvement, and initiating and managing change. To do this you will combine patient-facing, analytical and administrative elements of work.
Primary Responsibilities
* Carry out telephone and face-to-face consultations with patients and/or carers with medication queries ensuring accurate notes of consultations are recorded
* To provide medication review services via face-to-face, remotely or by telephone with patients/carers.
* Work with nursing staff and other community/Practice pharmacists for medication reviews for patients
* To provide advice and support for patients with self-limiting conditions
* Encourage cost-effective prescribing across the Practice
* Review all safety alerts ensuring the required actions are taken
* To reconcile medicines following patients discharge from hospital or clinic appointments
* To discuss with patients, if appropriate, any changes in treatment from discharge or clinic letters
* Review medications for newly registered patients
* Work with practice to ensure a robust repeat prescribing system
* To provide subject matter expertise on medication monitoring, implementing and embedding a system
* Carry out targeted medication reviews for high-risk patients
* To re-authorise medication for repeat prescribing within scope of practice
* Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required
* Review and implement safe prescribing systems for vulnerable patients, improving compliance
* Advise clinicians on any new guidance pertaining to medications
* Actively signpost patients to the correct healthcare professional
* Assist with Quality Outcome Framework targets
Secondary Responsibilities
* Partake in audit as directed by the audit lead
* Provide assistance with dispensary matters
* Provide education on medicines for the clinical team
* Review the latest guidance ensuring the practice conforms to NICE, CQC etc.
* Act as the practice point of contact for all medicines related matters, establishing positive working relationships

* There would be an expectation that you will work towards a prescribing qualification if you do not already hold this.
* Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individual's responsibility to remain up to date with recent developments.
* Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.
* Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.

* Develop and maintain a Personal Learning Plan.
* There is also the need to establish and maintain good liaison with other surgeries and agencies, including secondary care and commissioners.
Meetings
* It will be necessary to attend and contribute to various practice/Primary Care Network meetings as requested. The only reason for not attending will be annual, study or sick leave.
