The Reablement team assists patients requiringadditional support and rehabilitation on discharge from hospital, and those inthe community who need increased care or therapy input. Assessments areholistic and focused on maximising independence at every stage. Patients are atthe heart of everything we do.
This Post is Based in Wychavon and is offered on a fulltime basis, with flexible working and alternative hours considered.
Main duties of the job
As a memberof the reablement team, you will work closely with therapists and social careteams, having the unique opportunity to experience both sectors whilst with us.
You willperform holistic, goal focused, patient centred assessments, and developthorough treatment programmes to meet the goals. You will manage a variedcaseload, spanning all Occupational Therapy specialities.
You will workclosely with both health and social care colleagues, in a blended approach topatient care. Physiotherapists, nurses, occupational therapists, therapyassistants and social care staff undertake collaborative working on a dailybasis to ensure that all patient needs are met. Regular clinical supervisionand professional support are provided. We positively encourage any specialinterests which complement the patient demographic that we see
You will besupported in developing leadership skills and widening your Clinical knowledge tobe able to reach Band 6 Level through an individualised plan.
