This role is a new exciting opportunity for a Registered Paramedic within Your Health Partnership PCN. Candidates should have a portfolio demonstrating their advanced skills and a broad in-depth knowledge to support patients in primary care. You will work with a mentor to help support your learning needs and have access to supervision. In turn, you will be expected to engage in activities to evaluate and develop an area of practice together with actively contributing to the support of peers and colleagues.
Main duties of the job
As the Emergency Care Practitioner (Community Paramedic) within our PCN, you will be working alongside our Primary Care colleagues to provide the first point of contact for patients presenting with undifferentiated and undiagnosed problems utilising your clinical decision-making skills to diagnose and create management plans to support our patients. The focus of this role will be our housebound and aligned care home patients within our community beds team supporting our ACP and GP colleagues as a wider team.

Person specification
Experience
* Recent post qualification experience as a Band 6 (or equivalent) paramedic.
* Able to operate at an advanced level of clinical practice.
* Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
Qualifications
* Educated to degree level in Paramedicine.
* Completed consolidation of learning period as a newly qualified paramedic.
* Health & Care Professions Council (HCPC) registration.
* Non-medical prescribing.
* Pre-reg MSc in a training programme approved by the College of Paramedics.
You must have appropriate UK professional registration.
