A vacancy has arisen within the Unplanned Care Team, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess, plan, manage, and evaluate the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges.

Main duties of the job
The District Nurse Caseload Manager is a predominantly clinical role, with time split between 80% clinical and 20% non-clinical focus.
The Caseload Manager will be responsible for the day-to-day smooth running of the caseload and hold clinical responsibility for the caseload they are managing and leading on, ensuring patients are appropriately assessed and care is delivered in a timely manner with the involvement of patients and carers as appropriate.
The caseload holder will also ensure that team members are appropriately trained, supported, and have the right knowledge, skills, and competencies to meet the requirements of the role.
