Job summary
The Care Coordinator will play a vital role in support theeffective and efficient operation of the dispensary within a general practicesetting. Funded by the Additional Roles Reimbursement Scheme (ARRS), this rolewill focus on dispensing tasks, coordinating medication management, andsupporting the delivery of patient-centered care. The role aims to streamlinemedication services, improve patient access, and support the multidisciplinaryteam to ensure safe, timely, and high-quality service.
Main duties of the job
Assisting with the accurate and efficient preparation, dispensing, and distribution of medications.
Coordinate the management of repeat prescriptions, ensuring timely processing and effective communication with patients.
Monitor stock levels, reorder medications, and manage supply chain logistics
Provide personalised support to patients with medication-related queries, ensuring clear communication and understanding.
Assist patients in understanding their prescriptions, medication regimes, and any necessary adjustment
Collaborate with GPs, Pharmacists, nurses, and other members of the multidisciplinary team to coordinate patient care and enhance medication safety.
Liaise with community pharmacies and hospitals to facilitate seamless care transitions.
Maintain accurate records of all dispensary activities, including dispensing logs, patient interactions, and stock levels.
Use practice systems and software to log and manage patient information securely and in compliance with data protection.
Identify and report any discrepancies or issues with prescriptions, medications, or patient records.
Participate in audits, quality improvement initiatives, and patient feedback activities to enhance dispensary services.
Stay updated with best practices in medication management and support training for colleagues as needed.

This role is intended to become an integral part of the PCNs multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to provide an all-encompassing approach to personalised care and promoting and embedding the personalised care approach across the PCN.
There may be a need to work remotely depending on the requirements of the role.
Please note that the role of a care coordinator is not a clinical role
Key Responsibilities:
Dispensary Support:

3. Engage in proactive follow-up with patients to ensure adherence and resolve any medication-related issues
Care Coordination and Communication:

5. Act as a point of contact for the dispensary, coordinating between the clinical team and patients to address medication needs.

Essential
13. 1 - Strong communication and interpersonal skills with a patient-centred approach
14. 2 - Attention to detail and commitment to accuracy
Desirable
15. 1 - Undertaken a GPhC accredited Dispensary Assistant Course
16. 2 - Completed a Personalised Care Institute accredited two-day care coordination course
17. 3 - Proficient in using patient management and clinical systems
18. 4 - Experience of working in a healthcare or dispensary setting, ideally within primary care
