Job Title: Registered Nurse Associate 2 hour UCR
Band: 4
Salary: £ 26,530 - £29,114 per annum (pro-rata)
(Current NHS Service will be taken into consideration)
Hours: 37.5 Hours over a 7 day working week
An exciting opportunity has arisen to support Care Connexions as a Nurse Associate alongside clinicians in the 2 hour Urgent Community Response team.
You will be a Nursing Associate working within a multidisciplinary team and carry out a diverse range of duties to support and deliver the functions of the 2 hour Urgent Community Response team alongside Care Connexions. Care Connexions provides a single point of access to help those who are in a health crisis to avoid unnecessary hospital attendance or admission through timely triage.
As a Band 4 Nursing Associate within the team you will be regularly supported and supervised by a nominated senior clinician and you will be encouraged to take personal responsibility for clinical supervision and your continued professional development plan.
You will be required to undertake the administration of medications; however, these will not include medications deemed as high risk.
You will actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and Code of Conduct. You are required to maintain links with your professional body and provide evidence for the revalidation process.
You will provide support to the multidisciplinary team and ensure the effective assessment, formulation, planning and monitoring of care given to service users. Including involvement in the Care Connexions triage process.
