This role is vital in tailoring and implementing personalized care plans for patients in both clinical and community settings, ensuring each individual receives the attention they deserve. Embracing all facets of Continence and Stoma care, the post holder will embark on a journey of professional development supported by specialized training to achieve key competencies. Collaboration will be at the heart of this role, as the individual will engage with various agencies within the Local Care Organisation to foster a seamless, coordinated approach to patient care, while also championing education and resource sharing among team members and stakeholders.
