A fantastic opportunity has arisen for an administration based care coordinator to join the South Uttlesford PCN team, potentially on a full-time basis, but we will consider flexible working requests. The post-holder will form part of our multi-disciplinary, primary care team, providing care coordination across health and care services. Care coordinators review patients needs and assist them accessing services and supporting them in understanding and managing their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate. Main duties include: Working closely with GPs, practice teams, PCN team and wider system partners. Managing the coordination of care delivery across several PCN care delivery programmes. Acting as a central point of contact to ensure appropriate support is made available to them and their carers - to understand and manage their condition and ensuring their changing needs are addressed. Maintaining and updating EMIS Hub, including annual leave, appointments and reporting. Aiding the 4 practices with frailty, ECF and other core work. Communicating and working alongside the frailty nurses. Meeting timescales/deadlines for audits and written returns to ensure that the practice meets quality standards and receives the designated funding. The ideal applicant will have the following skills, experience and qualifications: Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups. Familiar with local resources and services, including how to access them. Motivated by helping people, with care and empathy. Effectively manages own time, workload, and resources. Strong listening and communication skills.
