The successful candidate will actively contribute to both high quality patient care and cohesive team working. They will act autonomously within their professional scope of practice providing care for patients face-to-face, telephone consultations and provide home visits. Main duties of the job The role of the Advanced Clinical/Nurse Practitioner will be to act autonomously within thier professional scope of practice in managing their patient caseload. They will provide care for their patients face-to-face, telephone consultation and provide occasional home visits. Working hours are negotiable to preference.
