Skills: Dementia, Experience or non-experience, Care in the community, Elderly, Learning disabilities, Drivers Job Duties: Provide compassionate care and support to individuals in the community, focusing on the elderly, those with dementia, and individuals with learning disabilities Assist with personal care tasks, including bathing, dressing, and grooming Administer medication according to prescribed schedules Monitor and record individuals' vital signs and report any changes or concerns to the appropriate healthcare professionals Assist individuals with mobility and transfers, using appropriate equipment and techniques Support individuals with meal planning, preparation, and feeding Engage in meaningful activities with individuals to promote socialisation and emotional well-being Provide companionship and emotional support to individuals Assist individuals in accessing community resources and services Ensure a safe and clean environment for individuals, including light housekeeping duties Maintain accurate and up-to-date documentation of care provided Collaborate with other healthcare professionals and family members to develop and implement individualised care plans Adhere to all relevant policies, procedures, and regulations Required Qualifications: Experience in caring for individuals with dementia, the elderly, or learning disabilities (experience preferred but not essential for non-experienced candidates) Valid driver's licence and access to a reliable vehicle Ability to work independently and as part of a team Excellent communication and interpersonal skills Empathy and compassion towards individuals requiring care Ability to follow instructions and care plans accurately Good organisational and time management skills Education: No specific educational requirements, but a relevant qualification in health and social care would be advantageous Experience: Prior experience in a care-related role is desirable but not mandatory Knowledge and Skills: Understanding of person-centred care principles Knowledge of common health conditions related to elderly individuals, dementia, and learning disabilities Proficiency in administering medication following proper protocols Ability to handle challenging behaviours with patience and empathy Knowledge of community resources and services available for individuals requiring care Preferred Qualifications: Relevant qualification in health and social care First Aid/CPR certification Additional training in dementia care or specific learning disability support Working Conditions: This role involves providing care in clients' homes and within the community. It may require physical exertion, including lifting and assisting with transfers. Flexible working hours may be required, including weekends and evenings. Personal protective equipment and proper infection control practises must be followed to ensure the safety of both the care worker and the individuals being cared for.
