The IV Service cares for patients in both clinic settings and their own homes throughout the West Sussex area.
The successful candidate will deliver high-quality care within a community setting, working towards patient rehabilitation, self-management, and independence. This role involves managing the day-to-day clinical needs of an identified caseload, undertaking initial assessments, and ongoing management of IV therapy., + Manage day-to-day clinical needs of an identified caseload under the guidance of the IV Nurse.
+ Conduct holistic assessments, develop care plans, and evaluate patient progress.
+ Act as an information resource for patients, their families, and multidisciplinary colleagues.
+ Empower patients to participate in their care, encouraging self-management and independence.
+ Ensure compliance with Trust policies and procedures, maintaining high standards of clinical documentation and data protection.
