We’re looking for a dynamic and experienced operational leader to take on the pivotal role of Specialty Manager within our evolving community-based frailty services.

As the national focus shifts from acute to community-based services, you’ll be at the forefront, driving service evolution and delivering outstanding care for older adults.
As Specialty Manager, you are part of our newly formed Integrated Frailty Leadership Team; together with a Clinical Director, AHP Lead and Matron, you lead the SBU and are a key member of the Division’s management structure, leading and driving our integrated frailty services, including:
1. Single Point of Access
2. Hospital at Home
3. Urgent Community Response
4. Intermediate Care
5. Milford Integrated Care Hub
You will also be working with services across the whole Frailty pathway into an acute setting, including:
1. Frailty Same Day Emergency Care
2. Acute Frailty Team
3. Inpatient wards at both acute and community hospital sites
The Assistant Specialty Manager will work closely and provide support to the Specialty Manager in the Integrated Frailty Strategic Business Unit (SBU). The Assistant Specialty Manager will also work collaboratively within the community management team. A key function of this role will be to manage the service data locally prior to wider submissions and to establish processes to ensure data is robust and fit for local processes. Key working relationships will include clinicians, service managers and wider internal and external stakeholders.
The post holder will assist with co-ordination of service improvement work to support service transformation and development working with clinicians and service managers. A key role is the day-to-day management of all administrative and clerical services within the allocated services. Working closely with the Specialty Manager to ensure that the services meet and achieve national performance indicators; i.e. Urgent community response 2-hour delivery, as well as all quality, activity and financial targets and indicators.
