We are looking for people to work as part of a small highly skilled and passionate team delivering a Stop Smoking Service, operating across Devon to support local residents to quit smoking through a 3-tiered approach service. The role is ideally suited to people with strong communication skills, who enjoy working with clients to support them to make improvements to their health. We support clients to give up smoking through delivering tailored stop smoking support, including carrying out initial assessments and interventions using proven behaviour change techniques in line with national guidelines. You will have a wide and varied caseload and will need to be able to prioritise multiple clients needs via multiple channels including in community settings, workplaces and virtually using digital media. This is a Full-time role. Core hours are Monday Friday 08:30am 5pm. Flexible hours are required to meet the needs of the service including evening/weekend work in agreement with the service lead. You will live in the local area of Devon to be able to travel and work daily in the community and the office premises. You will own a car and be able to drive to client sites and other offices if required. Key elements of the day-to-day role: Contribute to the assessment of clients for suitability for the service and eligibility for the right tier of support. To carry out CO testing using equipment provided. To assess nicotine dependence and readiness to quit using the Fagerstrom test. To support the client in making the best decision on pharmacotherapy/ Quit Aid. Provide NRT via a voucher scheme/direct supply. Ensure appropriate records are kept for all contacts with the service via the service CRM. Follow up and support clients to 4 week and 12-week quits. Support clients who fail in their attempt to re-engage with the service and make subsequent attempts. Seek out opportunities to develop and implement stop smoking services in a range of health and community settings. Complete all service paperwork in a timely manner in line with agreed protocols. Care/service/function provision: eg,** To deliver excellent levels of customer service and coaching support to people, and address or report, in a timely and proactive manner*. oTo plan and deliver smoking cessation advice and coaching support sessions, including advice on stop smoking medications, working within recommended protocols and guidelines. This will involve individual, telephonic, on-line and group work sessions in a variety of community settings and venues. oDevelop individually tailored support plans including recommending treatments based on assessed needs. oProvide information to people about stopping smoking and health-related issues in line with best guidance and protocols. oProvide general health and lifestyle advice and support to people using the service e.g. healthy eating, physical activity, stress management. This will include individual and group-based interventions, signposting and referring to other health, community, and voluntary services. oResponsible for the delivery of stop smoking support a range of clients as defined by the commissioner. oTo be involved in practice awareness raising sessions in Primary Care, Secondary Care and Community settings. oTo be involved in the marketing and promotion of the service alongside other colleagues. This may involve attending events on evenings/weekends. oUndertake specific project and/or development activities in conjunction with the Service Lead. oRespond effectively to referrals within stated timescales self-referrals from the individuals, and those from colleagues /other workers/GPs. oEnsure administrative duties are carried out efficiently and effectively in order to meet the needs of the service. oTo be involved in monitoring and evaluation of client feedback, offering recommendations as part of the team to improve services. Working as part of a Team:eg, ** As part of a team, deliver person centred care whilst meeting the service standards and objectives. Essential existing skills/experience: You will live in the Devon area. Understanding of behaviour change techniques. NVQ level 3 or equivalent in health-related studies OR substantial. Successful experience of delivering stop smoking interventions in a range of settings. Smoking Cessation Training Level 2 delivered by an NHS Stop Smoking Service or the National Centre for Smoking Cessation and Training (or to undertake within 1 month of starting in post). As this post is community-based, post holders will need to be able to independently travel with own car to be available to drive across the Devon area. IT literate and able to confidently use online methods of communication. You will also need to have a good eye for detail, and you will be accurate with data input and reporting. Customer facing experience ideally in health setting or service. Experience of using databases or digital systems to record data. Full training for all roles including becoming a NCSCT Stop Smoking Advisor will be provided along with a programme of continued professional development. Have we described you?
