This is an exciting opportunity for you to work within Strategic Commissioning to support our Joint-Funded Reviews project team, who will be doing collaborative, data-driven work with Health colleagues to undertake targeted reviews of people in receipt of Joint-Funded Support, to improve outcomes in a truly personalised way and ensure proportionate care and support is in place.
We are looking for dedicated Service Advisors with strong interpersonal and communication skills, and the ability to work flexibly and confidently. Your role will include liaising with other professionals, providing advice and guidance, and updating our case management system. You will have good skills in managing your workload demands and meeting deadlines.
You will have excellent IT skills, knowledge of continuing health care, community care services and relevant legislation, including the Care Act (2014), the Mental Capacity Act (2005) and Deprivation of Liberty Safeguards. Ideally you will have at least two years’ experience of care work, though this is not essential. You will have experience of working with the public and partners, keeping records and constructing reports.
The role will enable you to work flexibly through the hybrid working model, with the working week split between home and the office. NCC staff enjoy access to the latest mobile working technology, support and supervision. An induction and relevant training will also be provided.
