Job summary
Mid Yorkshire NHS Teaching Trust has been awarded new funding to implement and provide a dietetic-led Off-FP10 service for enteral (HEF) and oral nutritional support (ONS) across Wakefield Place. Working in partnership with Wakefield Health and Care Partnership and the ICS, this service will ensure all patients requiring nutritional support are referred to and reviewed/monitored by the dietetic team. This new development will provide a patient-centred, needs based service, which will allow seamless and more timely care for community patients who may require nutritional support intervention. The aim of the new service is to stop inappropriate prescribing of ONS (in line with NHS England intentions), ensure all patients receive timely and appropriate dietetic intervention and provide regular and ongoing training to all care homes across Wakefield Place. The service also aims to reduce health inequalities by ensuring improved access to dietetic support for all patients, regardless of location.
The service has been commissioned on a permanent basis, so we are looking to recruit into all areas including qualified dietitians, dietetic assistants and a new admin team.
Main duties of the job
This exciting new post will lead the implementation of the new service, providing support to other team members while maintaining their own discrete caseload. They will also work in partnership with Medicines Optimisation. The post holder will support the Service lead in the evaluation of the service, undertaking audits and providing reports as required. Excellent communication and team working skills are essential.
The post-holder will be based in Wakefield Community but may be required to attend meetings at one of the 3 hospital sites. The post-holder may also work remotely for part of the week.
The dietetic department is fully committed to CPD and clinical supervision. As we take A, B and C students from Leeds Beckett University, the post holder will also be expected to take an active role in supporting and supervising our student training program.

Job responsibilities
* Communicate highly complex patient related information effectively to ensure collaborative working with MDT members and with MDT colleagues across health and social care sectors to ensure the delivery of a co-ordinated multidisciplinary service. This will include case meetings and effective goal planning.
* Provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals, to promote understanding of the aims of dietetic intervention and to ensure a consistent approach to patient care.
* To lead the team in setting up, maintaining and evaluating a dietetic-led Off-FP10 model in the Wakefield community working closely with colleagues of all bandings.
* To develop, maintain and update the ONS Formulary to be used by all dietitians across Wakefield Place.
* Working with colleagues of all bandings to promote excellent nutritional care to people living within the Wakefield community, working closely with other professionals to plan, deliver and evaluate training packages and resources on a variety of topics such as Management of nutritional care in care home settings, Cardiac Rehab and wound care.
* Maintain own clinical professional development (CPD) by keeping abreast of any new trends and developments and incorporate them as necessary into your work.
* Be a member of the CPD programme by the attendance and delivering of presentations and training sessions at staff meetings, tutorials, training sessions in house and by attending external courses and practising reflective practice.
* Participate in the staff appraisals in line with the trusts values and behaviours, through regular supervision of staff of various bands to be able to help in the support and development of qualified and non-qualified colleagues within the team.
* Undertake the measurement and evaluation of your work and current practices through the use of evidence-based practice projects, audit and outcome measures, either individually or with Clinical Lead/Team Lead. Make recommendations for change and support in delivery of changes.
* To take part in clinical governance and risk management and apply to work situation.
* To lead the development and implementation of clinical audit plans as relevant to clinical specialism and contribute to department audit programme.

* Plan a care programme for individual patients. This may be in conjunction with other professional staff within the multidisciplinary team (MDT).
* Communicate effectively with patients and carers to ensure understanding of condition. Communication skills of persuasion, motivation and explanation to encourage patients to undertake their treatment programme.
* Use verbal and non-verbal communication skills to gain informed consent with patients where there will often be barriers to effective communication e.g. loss of hearing, altered perception, expressive or receptive dysphasia, pain, fear.
* Assess capacity, gain valid informed consent and have the ability to work within a legal framework with patients who may lack capacity to consent to treatment.
* Evaluate patient progress, reassess and re-advise accordingly.
* Manage clinical risk with own identified patient case load.
* Supervise, educate and assess the performance of nutrition and dietetic students; this would be to a graduate standard and involve working with universities to ensure the standard of practice and teaching meets standards set by the degree level qualification.
* To be involved in the continuous evaluation and development of resources used within the team such as diet sheets and training materials.
