To register patients for follow up in virtual clinic on HISS in a timely manner after discharge from hospital. To be aware of any overseas visitor or private patients that are referred and notifying relevant departments To be responsible for the timeliness, accuracy and integrity of the data which is recorded on paper and electronic systems. To ensure any and all actions are accurately reflected on both paper and electronic systems. To ensure that all patient records are maintained accurately on CliniTouch, Systm1, CITO, and any other relevant systems inclusive of that within the Microsoft office and google products used within the service. To maintain efficient office filing systems. Review, update/develop as required. To carry out general office duties including the processing of post efficiently and effectively, to action routine correspondence on receipt and forward to appropriate member of staff for review. Liaise and support virtual ward administrator for all services as required. To arrange team meetings when requested, using diary management and organisational skills to accommodate various staff across CMGs and external stakeholders. Support service with audits, metrics and service improvement projects as required. Be a visible, accessible and credible member of the SPIN team. Support the service leads with the process of change and new services, demonstrating tenacity, drive, professional integrity, balance and perspective. Motivate all team members, and display a supportive, confident attitude when communicating with all colleagues.
