Overview
The CFASS Level 2 advocate provides services aimed at enhancing the ability of children/youth with disabilities to function as part of a family unit and supports caregivers/families in their ability to care for the child/youth in the home and community. The role focuses on delivering educational resources, advocacy, and training to caregivers and youth to increase access to community services and maintain self-sufficiency.
Responsibilities
* Engage and interact with children/youth and caregivers to provide advocacy, support, and educational resources.
* Assist families/caregivers in accessing and utilizing community-based services independently.
* Educate and train caregivers/families on available resources and advocacy methods, empowering them to support the child’s needs.
* Provide guidance related to children’s chronic conditions or life-threatening illnesses.
* Deliver one-on-one or group training (up to 12 participants) to help youth and families develop behaviors that promote success in the community.
* Offer self-advocacy training for youth and families, including during community transitions.
* Work collaboratively with the family team to develop and implement CFASS service plans, in compliance with Medicaid and Home and Community-Based Services (HCBS) regulations.
* Maintain accurate records and document all interactions and progress in compliance with organizational procedures.
Work Conditions:
* CFASS services are provided in home or community-based settings, ensuring services are delivered in the least restrictive, most integrated environments possible.
* Must be able to work flexible hours, including evenings and weekends, as needed.
Qualifications:
* Minimum: Bachelor’s degree with two years of related experience in human services.
* Preferred: Master’s degree in education or a related human services field, with at least one year of applicable experience.
* Experience working with children and youth, particularly those with developmental or behavioral challenges, is highly desirable.
Training Requirements:
Mandatory training within six months of hire, including:
* Mandated Reporter Training (to be completed prior to service delivery)
* Personal Safety/Safety in the Community
* Trauma-Informed Care
* Suicide Prevention
* Domestic Violence signs and interventions
* Strength-based approaches
* Regular ongoing training is required to remain updated on best practices and safety protocols.
Job Type: Full-time
Pay: £25.00-£35.00 per hour
