Main duties of the job

Participation in outpatient or virtual clinics or home visits
Participation in teaching and training
Regular audit, data collection and quality improvement workto ensure ongoing improvements in the care of frail older people occurs
Maintain overview of all ward patients
Actively participating and facilitating discharges on theward
Support and supervise junior doctors on the ward
Liaise with Same Day Emergency Care ANPs if necessary
Contribute in the electronic and verbal handover of patients

Job description
Job responsibilities
This post isoffered on a full time (10 PA) basis although less than full time applicationswill be considered and the department has a mixture of full and part time consultantsin order to ensure a supportive environment for all. All our consultants workcross site but the successful applicant will work within the organizationpredominantly either at Pinderfields or Dewsbury and District Hospital. Thedepartment last year saw 9,403 admissions, saw 654 new patients in clinics andcarried out 844 follow up appointments.
TheElderly Medicine Department within the Trust consists of 15 substantiveConsultants. The Department is supportedby Associate Specialist, Specialist Registrars, Core Medical Trainees, VTSTrainees, FY2s and FY1s.
Clinicalresponsibilities will include:

This postis associated with a number of different specialty interests includingCommunity, Orthogeriatrics and acute frailty.
This postincludes on-call responsibilities as part of a dedicated Elderly Care on-callrota. Working arrangements currentlyconsist of the following:
Scheduled evening duties 5pm 8pm Monday to Friday currently2:18 seeing older patients (> 80yrs or > 65yrs from Care Homes orpatients with frailty) admitted via the Acute Care of the Elderly Unit variesfrom one or two patients to five/six patients
On-call weekends currently 3:18 (1:6) 8-hour resident oncall seeing approximately 20-25 patients with the support of the registrar. 1consultant on call at Dewsbury and District hospital and 2 at PinderfieldsGeneral Hospital.
Overnight On-call duties are currently undertaken on a 1:18currently.
The on call element of a 10PA job plan is assessed as Those on less than full time working have on call commitments depending ontheir preference of 50%, 75% or 100%.
Supporting Professional Activities in accordance with theTrust Framework, with at least aminimum of SPAs included in the core 10PA timetable. Additional SPA or DCCtime may be commissioned in line with the skills of the successful candidateand changing requirements of the service.
