Main duties of the job
Practice nurse role would include :
Health promotion and annual reviews for patients with long term conditions such as Diabetes, Asthma, hypertension and COPD. Other skills include - Phlebotomy, Wound care, Vaccinations, Smears and child immunisations.
Frailty nurse role would involve:
Visiting housebound patients to carry out urgent care plans and provide nursing support and referrals. Liaising with outside agencies and other health professionals to provide person-centred care. Attendance at monthly online touchpoint meeting with Harrow frailty team.

PRACTICE NURSE
Role Summary:
1. To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.
2. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
Primary Responsibilities:
3. Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health
4. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
5. Provide vaccination and travel medicine services
6. Provide wound care (ulcer /Doppler etc.) to patients
7. Request pathology services as necessary
8. Process pathology results as required
9. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
10. Maintain accurate clinical records in conjunction with extant legislation
11. Ensure read codes are used effectively
12. Maintain chronic disease registers
13. Develop, implement, and embed well woman clinics
14. Develop, implement, and embed well man clinics
15. Implement vaccination programmes for adults and children
16. Prioritise health issues and intervene appropriately
17. Recognise, assess, and refer patients presenting with mental health needs
18. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
19. Support the clinical team with all safeguarding matters, in accordance with local and national policies
20. Deliver opportunistic health promotion where appropriate

FRAILTY NURSE (EPN)
Role Summary:
The post holder will be responsible for the delivery of specialist practice nursing to patients who require additional care which falls outside the remit of the practice nurse. The focus of the role is to ensure patients, predominantly in their own home, are assessed swiftly and their urgent, and long term nursing needs are EPN will work collaboratively with other medical professionals in the community to meet the needs of patients.
