Job summary An exciting opportunity has arisen for a dedicated nurse to lead in the delivery of the Pre-Assessment service for the new Central Hampshire Elective Orthopaedic Hub, located on the Royal Hampshire County Hospital site in Winchester. The Hub is an exciting new venture supporting the delivery of elective orthopaedic care for patients across the region and is a partnership venture between Hampshire Hospitals NHS Foundation Trust and University Hospital Southampton NHS Foundation Trust. We are seeking a proactive, enthusiastic, and experienced pre-assessment nurse who is interested in delivering evidence-based, patient-centred care at what is an exciting time for the service.The delivery of high quality, effective and compassionate care is essential to our organisation. The post holder will play a key role in ensuring that patients are appropriately screened and triaged for suitability at the Hub, pre-assessed and optimised in advance of their elective surgery and will be expected to actively contribute the Hub peri-operative pathways using their knowledge and skills. Pre assessment is a pivotal part of the patient's pathway ensuring the patients are in their optimal health for their procedures. Main duties of the job To practice autonomously, managing and supporting a caseload of patients. To effectively triage patients for the Hub, working within protocols and guidance for all base Trusts to ensure excellent patient experience and reduce potential delays in the patient pathway. To act as a specialist practitioner, supporting the wider team in pre-assessments and being a clinical expert in known patient risk factors for elective surgery. To maintain and promote high standards of Professional Practice to ensure quality and safety of patient care, experience, and the patient journey. To support and actively contribute to the development and management of peri-operative pathways within the Hub. To actively promote service and quality improvement and to work collaboratively to support the Hub and surgical division quality priorities. To work closely with the Hub booking and administrative teams to ensure patient pathways are not impacted and that patients are identified as suitable for the Hub. To work collaboratively with the wider HHFT pre-assessment team to ensure a robust service. Acting as a line manager and team leader for other members of the team. To act as a good role model promoting the Trust values at all times. To support the team in ensuring that clinical services are delivered to a high quality and that all compliance requirements such as CQC and contractual requirements are achieved.
