Job summary We are pleased to offer a substantive 4 day a week (30 hours) Team Leader position for the Community Palliative Care Team overseeing the whole of South Derbyshire including Derby City, Amber Valley, Erewash, South Derbyshire and Derbyshire Dales. The Community Palliative care team provide specialist palliative care support for patients and those important to them in their own homes. The Team Leader of the Community Palliative Care Team (CPCT) will work under the guidance, supervision and direction of the Palliative Medicine Matron. Main duties of the job The post holder will take overall responsibility and accountability for the team of clinical nurse specialists (CNSs) and will provide leadership to the team. They are accountable for patient, public and staff experience. They will lead, offer expert specialist clinical advice and oversee the service to ensure a high level of quality care is delivered to the patients referred to the Community Palliative Care Team. The post holder will ensure that the delivery of agreed professional standards of care are maintained. They will ensure the team are leading and participating in education, audit and research. The post holder will require excellent communication skills and have a sensitive and empathetic approach.
