Medicines Optimisation Pharmacist Care Home Support Team
Band 6 (£37,338 - £44,962)

Locala Care Home Support Team are a dedicated team of nurses, pharmacists, a dietitian and physiotherapist working alongside General Practitioners (GPs), consultant geriatricians and the hospital team to provide enhanced clinical support to residents and carers. We aim to maximise the quality of life for people who live in elderly care homes across Kirklees and reduce unplanned admissions to hospital. Our team have specialist knowledge and skills in the care of older people and frailty.
The service is based at Mill Hill Community Health Centre in Huddersfield and supports elderly residents in nursing and residential homes across Kirklees.
An exciting opportunity has arisen for a pharmacist to join our team. Their role will be to conduct clinical medication reviews, provide advice on medicines usage, and deliver education to residents and care home staff. In addition, they will optimise prescribing, minimise wastage of medicines, enhance adherence with medication, reduce medication-related risk factors and facilitate avoidance of medicines related hospital admissions, plus ensure compliance with medicines policies and legislation.
The role will involve supporting care staff, liaising with community care teams, secondary care, therapists and other key stakeholders.
This role requires a motivated and dynamic person to work within this established service and it is essential that you are able to work as part of a multidisciplinary team. Experience of working with frail elders within a care home setting is desirable. There are opportunities to educate colleagues and support developments within the team. Good communication skills and flexibility are essential qualities.
