Belmont Recruitment are currently looking for an experienced Adults Care Worker to join Worcestershire Council's Adult Social Work Team on an initial 3 month temporary contract with the view to extend. This is a full-time role working 37 hours per week, Monday to Friday. Key Responsibilities: Engage with adults and carers to assess needs and develop creative support plans that promote independence. Work collaboratively with multi-agency professionals to ensure holistic care and support. Conduct meaningful conversations to determine eligibility under the Care Act (2014) and plan for the future. Identify and address safeguarding concerns and support safeguarding procedures. Promote access to community resources and build effective relationships with partner agencies. Support adults through crises and empower them to achieve positive outcomes. Maintain accurate records and participate in reflective practice and professional development. What We’re Looking For: Experience in health or social care, supporting adults with diverse needs, including disabilities, dementia, and long-term conditions. Knowledge of the Care Act (2014), Mental Capacity Act (2005), and safeguarding policies. Strong communication, assessment, and support planning skills. Ability to build productive relationships with agencies and prioritize workloads effectively. Level 2 qualification in health and social care (or equivalent).
