Who You Are You are a committed and flexible individual with an HND qualification or equivalent, or significant experience in delivering person-centered support. You work well under pressure, have excellent communication skills in English, and preferably in Welsh. You possess an understanding of homelessness legislation, social policy, child protection issues, and have been involved in supporting vulnerable adults. You are proactive in personal and professional development, and are committed to anti-discriminatory practices, customer care, and working in partnership with other services. What the Job Involves This role requires you to deliver comprehensive case management and support interventions for individuals and households at risk of homelessness or already homeless, in accordance with the Housing (Wales) Act 2014. You will be responsible for preventing homelessness through various 'reasonable steps', providing housing options advice, preparing personal housing plans, managing case records, and collaborating with partner agencies. You will also engage with the community through outreach and support activities, maintain positive relationships with private sector landlords, and assist service users with a focus on customer service, equality, and data protection compliance.
