The Virtual ward Sister/Charge Nurse role will work in collaboration with the Virtual ward team and community in reach team to support the delivery of a high quality of safe and compassionate acute care at home. This will include delivery treatments in the patients normal place of residence.
The Junior Sister/Charge Nurse will be expected to work with the community in reach team to actively identify patients occupying an acute hospital bed to continue their acute care at home and facilitate the smooth transition into the Virtual ward.
The role will include monitoring patients virtually utilising technology and video calls as well as working within the hospital at home element in delivering treatments in the patients home.
The successful candidate should provide professional leadership and management to ensure immediate care to the relevant client group both virtually and face to face and to work with the wider multi disciplinary team.
Main duties of the job
To effectively communicate and care plan, manage the day to day running of the virtual ward ensuring flow and effective safe discharge to ensure capacity for patients admissions to the virtual ward. Duties will involve virtual monitoring and early recognition of deterioration as well as delivering face to face care and treatment within the patients own home.
To provide clinical nursing leadership and expertise in the care of all patients and create an environment that values and supports staff to reach their full potential.
The in reach part of this role will include having professional and clinical discussions with the wider multi disciplinary team to ensure patients are reviewed to transfer to the Virtual ward at the earliest opportunity.

Detailed job description and main responsibilities
Practice within an ethical framework based upon respect for the well-being and safety of patients and clients.
To be responsible and professionally accountable for the assessment of care needs; development of personalised plans of care, implementation and evaluation of care without supervision.
Lead a team and co-ordinate the clinical area in the absence of the Clinical Service Lead / Service Delivery Manager and reinforce the values and standards of compassion, dignity and respect in care, leading the delivery of clinically excellent care.
Ensuring the planning and delivery of care is aligned to professional values of care, compassion and respect, act as a change agent, developing clinically effective practice through the effective utilisation and integration of evidence based practice.
Support the Clinical Service Lead / Service Delivery Manager in ensuring a culture of protecting patients from harm and deprivation of basic rights ensuring a safe and effective service with continuous quality improvement.
Promote a clean and safe environment for staff, patients and visitors by ensuring compliance with legislation, policies and protocols including health and safety, healthcare associated infection prevention, clinical governance, including risk management and critical incident reporting and root cause analysis.
Ensure a high standard of care and record keeping in accordance with Nursing and Midwifery Council, Health and Care Professions Council, national legislation and local standards. Demonstrating safe and effective care including; timely administration of medicines; effective observation of patients and demonstrate effective communication with the multi-professional team regarding patient care. Assess the risks involved in the care of patients and ensure utilisation of practices and protocols to minimise those risks, including safe use of medical devices. To monitor the standards of care and with the support of the Senior Nurse and take appropriate actions when standards fall below expected levels.
Report incidents and near misses promptly and appropriately and with the support of the Clinical Service Lead/Service Delivery Manager and take effective action to minimise future risk. Contribute to the development and implementation of evidence-based protocols, standards, policies and clinical guidelines.
