Chronic Disease Practice Nurse Forum Family Practice
Forum Family Practice is looking to recruit an experienced and motivated Chronic Disease Practice Nurse to join our fabulous nursing team. We are seeking someone who can work a minimum of 20 hours per week.
As a Chronic Disease Practice Nurse, you will play a vital role in delivering high-quality care to our patient population. You will be involved in the management of long-term conditions such as asthma, hypertension, COPD, and diabetes, as well as performing smears, baby immunisations, travel vaccinations, and blood tests.
We use SystmOne (S1), and experience with this clinical system is desirable.
Main duties of the job
* Key Responsibilities:
o Deliver a high standard of clinical care and support the provision of enhanced nursing services, including chronic disease management, health promotion, immunisation, and screening programmes.
o Work collaboratively within the wider practice team to meet the needs of our patients, using evidence-based care principles.
o Be accountable for ensuring that agreed professional standards are maintained and reflected in clinical practice, supporting the implementation of policies and procedures.
o Promote a learning environment for staff and students, participating in education and training programmes as required.
o Frequently work unsupervised and be accountable for the care delivered, demonstrating sound clinical judgment, critical thinking, and autonomous decision-making.

Job responsibilities
Main duties:
* Management of long-term conditions: involvement, implementing and using protocols and clinical guidelines.
* Work with patients to support compliance with, and adherence to, prescribed treatments.
* Health promotion and NHS health checks.
* Urinalysis.
* Venepuncture according to local guidelines.
* Blood pressure / hypertension reviews / Ischaemic Heart Disease reviews.
* Routine adult and child injections and immunisations.
* Initiate, perform and interpret diagnostic procedures as appropriate for chronic disease management.
* Women's health checks including contraception, sexual health and cervical cytology.
* Support recall procedures, appropriate literature and health promotion, and audit.
* Ability to monitor and manage maintenance of stock and equipment to include refrigeration, single use equipment and general treatment room equipment.
* Maximise Quality Outcomes Framework income.
* Experience of audit and research.
* To take responsibility for own professional development and undertaking training for specific clinical tasks.
* Attend mandatory training courses and updates and any study days as appropriate to post.
* Participate in yearly appraisal, personal development plan and revalidation.

Person Specification
Experience:
* Able to demonstrate enthusiasm and commitment to professional development.
* An understanding, acceptance and initiative and common sense.
* Ability to work without direct supervision and determine own workload priorities.
* Can work well in a team and autonomously.
* Ability to work as part of an integrated multi-skilled team.
* Adherence to the need for strict confidentiality.
* Able to maintain accurate records.
* Flexibility of working hours/able to cover for nursing colleagues when required.
Qualifications:
* Registered RGN.
* Recognised qualification/training of chronic diseases.
* Asthma/COPD/Diabetes.
* Understanding of QoF and Chronic Disease Management.
* Keyboard and computer skills.
* Experience of SystmOne and Ardens.
