We have a fantastic opportunity for an Enhanced Clinical Practitioner (ECP) or Trainee Enhanced Practitioner to work in the Derby City Community Service, working part-time 30hrs over 4 days.
We will positively consider applicants who fulfil the Person Specification, with the exception of non-medical prescribing and advanced clinical decision making/patient examination skills.
However, we will consider those applicants who do not meet this criteria and are willing to accept a Band 6 Trainee Enhanced Practitioner role until successful completion of both programmes and the completion of a training portfolio.
The role will be either Band 6 or Band 7 (dependent on experience and qualifications).
The Derby City Community service works collaboratively with Joined Up care Derbyshire, in line with the Ageing Well Agenda, providing holistic, proactive care to residents in care homes improving residents' health outcomes with the support of an aligned General Practitioner, as part of the Enhanced Health in Care Homes model.
Main duties of the job
Enhanced Clinical Practitioner (Community) within Care Home Team:
Providing support to both the residents, relatives, and care home staff adopting a holistic approach providing health education/promotion, proactive clinical assessment, care planning, annual assessment, and long-term conditions management.
You will work as a key member of the multi-disciplinary team, working alongside General Practitioners, Community Pharmacists, Care Coordinators, Community Psychiatric Team, Physiotherapists, Occupational Therapists, and Social care providing joined-up care.

Job responsibilities
The role within the Care Home Team involves supporting a number of care homes, both residential and nursing, within a Primary Care Network (PCN).
It will require a high level of clinical knowledge and the ability to implement proactive care planning, education and working as part of the wider MDT.
