Overview
We are seeking compassionate and dedicated female Support Workers to join our team.
Responsibilities:
1. To provide care across two adult clients with differing needs in conjunction with additional team members.
2. To provide care in accordance with best practice and legislative requirements, reflecting policies and procedures and agreed standards under the direction of the Case Manager / Care Co-ordinator.
3. To support and enable service users to maintain skills and personal interests while delivering person-centred care unique to each individual.
4. To maintain skills at a current level and undertake such training and development as required, from time-to-time, to maintain and progress knowledge.
5. To provide all aspects of personal care, manual handling, medication administration, and communication support.
6. To collaborate with families and therapists to deliver tailored, person-centred care and rehabilitation.
7. To provide 24/7 support, including wake nights, sleep nights, and day shifts, demonstrating flexibility.
8. To monitor and record clients' health and rehabilitation progress using electronic care records and EMAR.
9. To drive wheelchair-adapted vehicles for client appointments.
