An exciting opportunity has arisen for an enthusiastic Band 5 Community Registered Nurse to join our Community District Nurse teams in Solihull.

The role involves working autonomously and requires a car driver/owner.
Main Duties of the Job
1. Work collaboratively with GPs and other professionals to ensure the best possible care is provided to patients in the community, avoiding unnecessary admissions and prolonged hospital stays.
2. Communicate effectively with patients, families, and carers, intervening to overcome barriers to communication, using advocacy skills when necessary to achieve effective outcomes for patients who may have sensory impairment, confusion, and/or aggression.
3. Assist the caseload holder in planning and ensuring work is allocated on a daily basis, prioritising as the service requires, utilising the dependency scoring tool.
4. Recognise and appropriately address risk factors to clients, carers, and Trust staff within their working environments.

Qualifications
* Current registration as a First Level General Nurse
Experience
* Knowledge in Safeguarding issues/procedures
* IV Therapy Competent
* Previous Community nursing experience
* End of life care
Additional Criteria
* UK Driving licence / car owner
* Ability to work shifts
* Flexibility to meet the needs of the service
