Ensure the provision of high quality, safe and effective patient care Work collaboratively as a key member of the team, promote a positive working culture, encouraging staff participation and involvement in developing and improving the service Actioning inbound referrals appropriately Dealing with queries from patients / family members Updating patient records during MDT meetings with plans Coordinating with the clinical team for any follow ups Liaising with the hospital teams regarding patient care Liaising with practices regarding patient care Processing discharges once patients care has ended Report any issues/ incidents/complaints/escalations to the Service Manager Other responsibilities include, but are not limited to: To undertake any other duties commensurate with the post and to react to statutory, legislative and NHS policy within the scope of the role
