Lead Frailty Nurse Practitioner in Primary and Community Care

An exciting opportunity to develop a Frailty Nurse Service for patients living in North Powys has become available. The post is a 2 year fixed-term contract working in rural and urban community settings covering Montgomery, Newtown, Llanfair Caereinion and Welshpool.
The role will coordinate and provide essential additional support to some of our most frail patients, supporting the development of expert frailty services in the community. The role demands thorough clinical assessment skills and a person-centred approach to formulate personalised comprehensive care plans which aim to keep patients at home for longer, reduce acute admissions and prevent hospital re-admission.
The postholder will be an autonomous clinician with excellent clinical and communication skills, using their experience and expertise of working with frail elderly patients in a community setting, to navigate the provision of care for the patients.
As a Band 8a nurse, you will be instrumental in managing a small team providing care and support to those with complex medical and social issues.
Main duties of the job
1. Assist the Primary and Community Care service leads in developing and ensuring robust and sustainable delivery of Primary and Community Care Frailty Nursing services.
2. Be a strong visible clinical leader who will support and shape the delivery of the Frailty Nursing service across several clinical teams, including Nurses, non-registered Health Care Workers and Administration staff.
3. Work with Primary and Community Care Nursing services leads to provide clinical leadership and direction to Frailty Nursing services through clinical supervision and development of services.
4. Responsible for equitable delivery of Frailty Nursing services to patients across North Powys PTHB.
5. Initiate the planning, evaluation and audit of practice, clinical pathways and protocols for Primary and Community Care Frailty Nursing Services.
6. Provide expert leadership and support in the assessing and treatment of complex patients, providing advice, guidance and teaching for less experienced staff across the north cluster of PTHB.
7. Ensure the standard of Frailty Nursing services meets Clinical Governance requirements as specified by PTHB and other recognised local and national standards.
8. Monitor the implementation of Standards for Health Services for Frailty Nursing across Powys North Cluster participating practices and to ensure all operational teams are aware of developing local and National actions.

Person Specification
Aptitude and Abilities
* Evidence of specialist knowledge and application of current best practice
* Resolution of professional/ethical issues
* Ability to work under pressure and set priorities
* Understands current policy and practice and implications for Community Frailty Nursing Service
* Ability to coordinate intra and inter professional projects
* Experience of critical appraisal of research and best practice guidelines, with appropriate application to clinical work
Qualifications and/or Knowledge
Experience
* Evidence of advanced level study and advanced level practice (including Level 7 clinical examination, clinical diagnostics and v300 independent nurse prescriber qualifications)
* Expert knowledge/previous experience working with frailty
* Live registration with the NMC as Registered Nurse/degree in Nursing or other health related subject or equivalent degree level relevant experience
* Management / Leadership qualification or equivalent preparation
* Knowledge of current Nursing issues within NHS care systems
* High level of knowledge of NMC code of conduct and professional standards / guidance
* Experience of taking a clinical lead role within an appropriate setting
* Evidence of planning and decision making
* To have managed complex patient situations where negotiation and clinical reasoning were necessary to result in an appropriate outcome
* Experience of critical appraisal of research and best practice guidelines, with appropriate application to clinical work
* Ability to speak Welsh
* Ability to coordinate intra and inter professional projects
* To have acted as a Mentor / coach for other person within the profession
