As a Registered Nurse (Band 4), you'll be an integral part supporting some of our most vulnerable patients and planning care for people with complex and challenging health needs. You'll be confident in all aspects of primary care delivery - including undertaking health assessments and performing diagnostic tests - and able to work flexibly to meet the needs of our service. The post holder will have completed their Nursing Associate training and be registered with the NMC, working independently under the leadership of registered nurses, working within the sphere of nursing and care, and within all aspects of the nursing process.
Main duties of the job
This role will complement the role of the GP and other Health Professionals as an integral part and key member of the care team. To make the best use of clinical skills within the practice in order to promote well-being and maximise health gain for patients. To provide a safe, high quality and accessible service to patients located at HMP Kirklevington Grange.
Job responsibilities:
1. Provide continuing care to patients across the practice population.
2. ECG, Phlebotomy, Urinalysis.
3. Assessment and planning of delegated care.
4. Monitoring of chronic conditions and well-being of patients including escalation of interventions when necessary.
5. Administering of injections, where necessary, in accordance with regulatory guidelines for Nursing Associates.
6. Assist with the implementation and monitoring of clinical care and monitor outcomes.
7. Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment.
8. Be able to perform cardio-pulmonary resuscitation according to current UK guidelines.
9. Ensure infection control guidelines are maintained.
10. Communicate effectively with other members of the primary health care team.

Person specification
Education
* NVQ Level 4 in Health and Social Care. Registered with the NMC and have the Associate Nurse qualification.
Knowledge and Experience
* Experience of working with people with complex mental health needs.
* In-depth knowledge of health of primary care, mental health and addiction support and an ability to work across these areas.
* Experience of work in a multi-disciplinary team setting.
* Knowledge of nursing procedures and practices.
* Previous experience of using or accessing mental health services or caring for someone who has accessed services.
* Knowledge or experience of working within a prison environment.
* Previous experience of working with people accessing Substance Misuse services or able to demonstrate an understanding of clinical pathways for substance misuse.
* Experience of supervising others.
