An exciting opportunity has arisen within the Review Team.
The Review Team is a multi-disciplinary team with members from backgrounds that incorporate social work, registered mental health nursing, and occupational therapy, who review individuals who have a funded package of care. Where appropriate, the team will look at lesser supportive packages to enable increased independence.
Main duties of the job
1. Completing reviews of packages of care funded by Manchester City Council and Manchester Integrated Board.
2. Facilitating move on from funded placements where appropriate.
3. Working in a strength-based way, so independence is promoted, and resources maximized.
4. Working in accordance with key legislation e.g. Care Act (2014), Mental Health Act (1983), Mental Capacity Act (2005), and Human Rights Act (1998).

Detailed job description and main responsibilities
Communication
1. Demonstrate high-level communication skills.
2. Utilise highly developed interpersonal skills including specific skills to facilitate effective communication when there are barriers to understanding e.g. sensory or cognitive impairment or in hostile and highly emotive situations.
3. Make reasonable adjustments to communication when these are indicated.
4. Establish and maintain robust communication networks with service users, carers, and other health and social care workers and numerous external agencies.
5. Have good written communication skills and be able to produce reports to Court.
Clinical Care, Governance and Quality
1. Work in a strengths-based way, maximising independence, and promoting wellbeing.
2. Assess complex mental health and social care needs of service users and carers, and the provision in place aimed at meeting this need.
3. Assess and plan in line with the requirements of the Care Act (2014), s.117 of Mental Health Act 1983 (as amended by the Mental Health Act 2007), Mental Capacity Act 2005.
4. Apply theoretical knowledge and understanding of psychology, sociology, psychiatry, and physical science to develop a case formulation describing an individual’s strengths and challenges.
5. Produce a support plan for providers which meets the desired outcomes of the individual but also maximises independence.
6. Commission/implement the right level of package of care at the right time so that independence is promoted and resources maximised.
7. Prepare applications to funding panel following review.
8. Produce court reports for the Court of Protection.
9. Convene and attend meetings in accordance with s117 and CPA requirements ensuring health and social care aftercare provisions and legislative requirements under s117 are met.
10. If qualified AMHP- Act as an Approved Mental Health Professional (AMHP), balancing the human rights of the service user within a legislative and statutory framework alongside the issues of risk and safety to achieve the least restrictive alternative.
11. Where applicable, make referrals to Client Financial Services.
12. Actively access and evaluate ongoing up-to-date research to underpin, develop and inform clinical practice.
Financial and Other Resource
1. Exercise good personal time management, punctuality, and consistent reliable attendance.
2. Maximise access to resources across the provider market and utilise universal services wherever possible.
3. Ensure mechanisms are in place to enable Fairer Charging.
Learning, Education and Personal Development
1. Participate in regular formal managerial and professional/clinical supervision and a shared appraisal of work performance with the line manager.
2. Engage in continued professional development relevant to the role and support the continued development of others through training and supervision.
3. Contribute to the delivery of multi-disciplinary training and induction of new staff.
4. Act as a Practice Educator for students within the sphere of professional qualification.
5. Supervise team support staff and develop and oversee support plans which they will carry out aimed at promoting independence.
6. Attend Professional Forums.
7. Maintain Professional Registration.
Safeguarding Children and Vulnerable Adults
1. Adhere to the principles of the Care Act in relation to the safeguarding of adults with Care and Support needs.
2. Raise referrals and contribute to multi-agency safeguarding planning meetings.
3. Work in an empowering and proportionate way.
4. Contribute to the development of an agreed protection plan enabling desired outcomes to be achieved.
5. Work in line with Manchester Safeguarding Partnership and GMMH safeguarding policies.
6. Identify safeguarding concerns for children and young people and liaise with Children’s Social Care, Early Help to ensure information is shared and appropriate action taken.
