Liquid Personnel is currently recruiting for an Adults Social Worker to join a busy Hospital Discharge Team based in Rochdale.

Your main responsibilities as a Social Worker:
* Undertaking section 2s and 5s.
* Facilitating hospital discharge
* Carrying out assessments in different wards.
* Managing a busy caseload within an acute hospital setting.
* Working closely with health colleagues.
* Ensuring adults in hospital are discharged in a timely and appropriate manner.
* Attending multi-disciplinary meetings.
* Mental capacity assessments leading to best interest decisions.
* Advising patients and families who do not meet the eligibility criteria and redirect them to other agencies.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
