Job summary The main duties of the posts are to assess and review individuals with complex health care needs within the community. Main duties of the job A vacancy have arisen with the Complex Care Team for experienced Registered Nurses (RGN, RMN or RNLD) to undertake the role of Complex Care Nurse Assessor, based in Cwmbran.The role will require the post-holders to undertake in-depth expert / specialist nursing assessments of individual residents in nursing homes and hospital sites across the five Locality areas of Gwent, on behalf of Aneurin Bevan University Health Board, to ascertain individual's eligibility for Continuing NHS Healthcare (CHC), Discharge to Assess and entitlement to NHS Funded Nursing Care (FNC) funding. Consequently, the roles will require the post-holder to facilitate the Multi-disciplinary Team assessment process, and to monitor the individual's changing needs and agreed packages of care within the homes.
