If youlove working in a community setting or want to start a new career pathway incommunity nursing, we have a great opportunity for a Band 5 Community Nurse tojoin Chippenham Community Team.
Theteam is based in Chippenham Community Hospital and provides patient carebetween and hours. Our core working hours are withshift patterns including some weekends, early and late shifts although thesecan be flexible. This post is for 26 hours over 3 or 4 days.
Thisrole is very rewarding and will include supporting a caseload of patients andproviding effective nursing interventions in patients homes and othercommunity settings using evidence-based practice.
Main duties of the job
As aCommunity Nurse you will be supported to gain the competencies to provide awide range of nursing interventions. Our clinical caseload includes patientsrequiring palliative care, tissue viability interventions and long-termcondition management and we encourage our nurses to extend their skills anddevelop specialist interests. The role is varied and interesting as we managepatients with complex needs and provide a timely response to unplannedreferrals.
Applicantswill need experience in NHS patient care and evidence of professionaldevelopment alongside excellent clinical and communication skills. The role canbe demanding but valuable and we are looking for a nurse, who is enthusiastic,motivated and who can work flexibly. We are a busy, friendly team, who would welcomethe opportunity to work with a new member of staff willing bring new ideas andexperiences to help us to develop our team and service.

1. Registered Nurse
2. To possess a current, valid full driving licence
3. Evidence of continuing professional development
4. Knowledge of legislation affecting professional practice
5. Experience of applying research based information to practice
