The successful applicants will provide essential support to our services and contribute to the delivery of high-quality, person-centred care to patients with mental health conditions, learning disabilities, and other complex health conditions. You will be responsible for providing support in a variety of settings, including inpatient wards and community services, on a flexible, as-needed basis.

Key Responsibilities
* Assist patients with daily living activities, promoting independence and dignity.
* Support the nursing team in delivering safe and effective care to patients.
* Observe and report on changes in patients' physical and mental health, ensuring their safety and well-being.
* Provide emotional support to patients, ensuring they feel valued, respected, and understood.
* Assist in maintaining a clean and safe environment for both patients and staff.
* Help with the administration of care, including supporting patients with medication and personal care needs where required.
* Contribute to the implementation of individual care plans.
* Maintain accurate patient records, adhering to Trust policies and procedures.
* Work flexibly to meet the needs of the Trust, which may include nights, weekends, and bank holidays.
