A permanent vacancy has arisen for a Band 5 UK Registered Nurse. This post is based in the GP Unit, caring for a wide variety of patients with wide-ranging health care needs to include rehabilitation, step down from acute hospitals, palliative care and long-term conditions.
We are looking for an individual who is forward-thinking, with excellent communication skills. The successful applicant will work closely with the multidisciplinary team who are dedicated to delivering safe and effective person-centred care and rehabilitation. Applicants will demonstrate the ability to assess, plan and implement care for our patient group, including older people.
This is an exciting opportunity, as we implement new models of care within the GP unit that will support the delivery of high quality, person-centred, safe and effective patient care. The post will also support the delivery of the National Dementia Strategy in its local context.
You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the UK NMC Register.
