We are looking to recruit a Band 7 DSN to join our dynamic team. We are a multidisciplinary team consisting of Consultants, Dietitians, Podiatrists as well as Specialist Nurses. The team receives referrals from many primary care and community services as well as local acute hospitals.
We are a unique community provider as we manage people with Type 1, Type 2 and other diabetes sub-types. We initiate Freestyle Libre and Dexcom according to local guidelines. We also deliver DAFNE & DESMOND structured education and are a VICTOR pilot centre.
We are based in Morden (at the end of the Northern line) and work within the London Borough of Merton. Morden Underground station is just around the corner and Wimbledon Station 1.5 miles away. We are also on many bus routes, with a tram station very nearby.
Minimum Requirements:
1. Relevant post registration training and experience in the management of patients with Type 1 & Type 2 Diabetes.
2. Ideally be an Independent Nurse Prescriber or demonstrate a willingness to achieve this qualification.
3. Be a great team player who is willing to work collaboratively with the multi-disciplinary team to deliver the best care possible for our patients.
4. Have a strong commitment to the delivery of high quality, safe, patient care and be a good role model in reflecting the Trust's core values.
5. Be competent to offer guidance and supervision to members of the clinical team.
