1. Provide professional advice and guidance for clinical therapy staff, clinical teams on effective assessment, planning, implementation and evaluation of patient care according to changing health care needs, ensuring that patients, relatives/carers are involved in all aspects of care. 2. Initiate and monitor changes and improvements to therapy practice in terms of patient flow, goal setting and discharge planning according to service plans and objectives, including ensuring that the requirements of new guidelines e.g. NHS Long term plan are implemented. 3. Support therapy clinicians in developing advanced clinical skills in assessment, and treatment, working collaboratively and co-operatively with multidisciplinary team/agencies to ensure that patient needs are met, especially in relation to on-going care needs and discharge arrangements. 4.Provide advice and support for other health and social care professionals, by being visible and accessible, acting as the first point of call for information and concerns referred on from the care community, liaising with other staff, clinical teams, patient services and external agencies as required. 5. Provide advice and guidance to the head of quality, nursing and Professional leadership on the provision of high quality evidence based therapy services including Podiatry, Adult and Childrens physiotherapy, Occupational therapy, Dietetics, Stroke service, MSK across all areas of CCICP. 6. Collect, collate, analyse and report information, ensuring that accurate patient records/documentation are maintained, taking action to address issues and solve problems. 7. Promote the individual needs of the patient, by acknowledging preferences, rights and choices, respecting privacy and dignity, and by promoting personalised care, anti-discriminatory practice, including taking action on inappropriate behaviour, alerting senior staff if patient care appears to be disrespectful or discriminatory. 8. Establish and maintain effective communication, maintaining confidentiality of information. 9. Continually monitor standards, by identifying risks, benchmarking, audit and research, ensuring that ward action plans are prepared and implemented to improve quality of care and services. 10. Monitor systems and processes for ensuring the effective management of risk, accurate recording and reporting, and maintenance of all records and documentation. 11. Analyse clinical risk reports/complaints, develop, implement and monitor action plans to address specific issues and shortfalls, reporting on improvements. 12. Monitor the implementation of policies, procedures and clinical guidelines, ensuring that reviews take place, including influencing improvements to care provided by other clinical teams and external agencies. 13. Ensure compliance with Trust policies, procedures and clinical guidelines for self and others, alerting senior staff if practice appears to contravene policy, or if concerned about any aspect of patient care. 14. Provide leadership on making changes and improvements in quality of care and development of clinical teams, ensuring that plans are implemented by monitoring and reporting on the impact of change 15. Act as a role model by demonstrating leadership and expertise, and by maintaining credibility within central Cheshire Integrated care Partnership and Mid Cheshire Hospital Foundation trust, the wider health care community and external agencies, ensuring a positive image of the Trust is maintained. 17. Develop and maintain systems and processes to ensure that work is appropriately prioritised and allocated, and that effective supervision is in place to support work delegated to staff. 18. In conjunction with the Head of Quality, Nursing and Professional leadership develop and maintain systems and processes to ensure that staff appraisal is in place for all staff in the service. 19. Support the Head of Quality, Nursing and professional leadership by ensuring that recruitment, staff induction and orientation, and mandatory training take place according to Trust policy. 20. Support CCICP by the effective use of resources, e.g. staff, budget, premises, equipment, supplies and materials. 21.
