Neasham Road surgery has a new opportunity for a Care Coordinator to join our existing team. The successful applicant will assist the practice staff in ensuring patient care is of a high standard at all times. Neasham Road surgery is a busy but great place to work. We have plenty of support to offer and opportunity to 'get stuck in'. We are looking for a similar minded candidate to join us in ensuring our patients receive the best of care. Please note this post is not a clinical one but an administrative patient liaison type post.
Main duties of the job
The main duties of this post will be to:
Assist in the planning and running of monthly MDT meetings
Creating reports to assist with the inviting in of patients for regular reviews and NHS screening programmes
Contacting patients in regard to their care and follow up
Supporting the practice in seasonable vaccination schemes
Supporting the practice and their frail and vulnerable patients, specifically coordinator frailty reviews
General support to all clinical and non clinical staff within the practice

Neasham Road surgery has a patient list size of 13,500 registered patients with 5 clinicians (3 GP Partners, 2 Salaried GP's) and 3 Clinical Practitioners. We have an excellent support team of nurses and health care assistants, nurse associate and a Pharmacist and Pharmacy Technician. We are a busy practice who provide high quality care to patients but believe in teamwork; with everyone from clinicians to the admin staff, working together. We wish to extend this care to patients by providing another route of support in this new post. Job responsibilities
Assist with queries from patients relating to issues other than booking appointments (such as queries about hospital appointments, help with signposting, etc)
Assist the IT team in online access to medical records and NHS appt
Assist the IT team with QOF and invitations for patients, specifically childhood immunisations and cervical screening
Support the practices frail and vulnerable patients, supporting frailty reviews and coordinating MDT and palliative care meetings
Support the practices ethnic minority patient groups - possibly by setting up a group to assist with translations, national screening programmes etc
Support the practices large community of travelling population; specifically around childhood immunisations and other vaccination programmes including that of cervical screening and other national screening programmes
Support the practice Safeguarding team with requests for information
Support the practice in seasonal vaccination programmes, including the organising of care home and housebound vaccinations
General support as and when required.

Qualifications
* GCSE Grade A - C in English and Maths
* Qualified to NVQ level 3
Experience
* Excellent communication skills, written and oral

* Hard working, adaptable and reliable
* Willing to work flexibly if needed
* Satisfactory DBS check
