Collaborate with IT to ensure full understanding of QOF recall systems and to ensure smooth running of clinics. Collaborate with palliative care services and other professional disciplines to provide a seamless service for patients who are end of life. Assist in the management of patients in the terminal phase by joint working with community/neighbourhood care healthcare professionals and palliative care services following end of life discussions and offer appropriate specialist opinion in relation to the management of respiratory illness. Assist to implement nurse led clinics for respiratory management using advanced specialist skills and knowledge to ensure consistent high standards of care. Contribute to PCN meetings and Projects.
