Dudley MBC are implementing a Short Term Assessment and Reablement Team (Dudley STAR Team) An exciting new team is being formed to operate the council’s new adult social care short term assessment and reablement service. The service will be offering local people an efficient and effective support package post hospital discharge, while supporting the NHS to be more effective and increasing their capacity. Following a hospital discharge and once home, the new team made up of occupational therapists, physiotherapists and support staff will assist people to regain their daily living skills, mobility, and independence at home, through a tailored bespoke ‘reablement’ therapy plan. The team will work closely and intensively with each person to meet set goals and outcomes over an agreed period. The focus will always be on promoting independence. Social work / care management and supervisory roles, care and support worker roles, along with physiotherapy and occupational therapy roles are being recruited to for the new team. All roles will have a clear career progression pathway within Adult Social Care, and training and development packages. The role We are looking for passionate, caring people with a genuine desire to support our vulnerable adults in the community and enable them to lead as independent a life as possible. This role will require you to support people in crisis or at times of ill health following a hospital visit and ensure they go home with the correct support to meet their needs. You will work with colleagues to plan and provide person-centred care that promotes respect, dignity and personalisation. We expect Home Care Assistants to ensure high quality care is delivered to service users, helping with their physical and personal needs including the administration of medication, if required. You will also need to be an effective communicator both verbally and in writing, with excellent recording skills. You will be expected to work as a member of a team. Relevant mandatory training will be provided during the induction period. As a Home Care Assistant you will be working contract hours per week on a set rota which includes weekends and some bank holidays, for which relevant enhancements will be paid. Hours of work are between 7am to 10.30pm, depending on the set rota pattern you are allocated and the needs of the service. You will need to have the right to work in the UK. DMBC do not offer sponsorship. You may need to work flexibly, travelling across the Dudley borough and will therefore need to have the use of a vehicle.
